Questionnaire for the 2006 Online Yearbook

The following are suggestions of information about you that could be sent to the committee in a variety of ways:

a word processing document file attached to an email
a handwritten or typed document sent through the postal service
Any of the above are acceptable and welcome!! For ease in filling out the form - print the page on your printer, send the page to yourself as an email or highlight and copy this page in it's entirety and paste it into a word processing document. You will then be able to add the relevant data and send it back.
Please see the "add yourself to the year book" section for additional information on to whom it should be sent and instructions regarding new pictures you would like to have included.

We would like to include additional sections this year which will highlight the memories we have from our years at West High. These may be included on each classmate's individual page rather than collective lists of the following in separate sections:

Favorite teacher (or teachers) and why
Favorite extracurricular activity (or activities) and why
Most memorable aspect of your West High Experience
Favorite memories or thoughts about classmates who are no longer with us
Clothing or dress fads you loved or hated
Favorite songs
Favorite photos - please send digital files (if possible)
OR ones you can wait to get back at the reunion dinner
OR a stamped, self-addressed mailer.
